Editor's note

During the epidemic period, medical workers, volunteers, community workers and patients in the first line of epidemic prevention suffered tremendous psychological pressure for a short time. At present, with the sharp reduction of the number of cases of new crown virus infection, psychological support has entered the vision of treatment strategy. In fact, psychological support intervention is not too late in the new crown pneumonia treatment. As early as January 26th, the state had urgently introduced the emergency principle of psychological crisis intervention. Prior to that, the National Health Protection Committee dispatched 150 psychiatrists from Shanghai, Sichuan and Hunan provinces to help Wuhan. At the central level, it is also indicated that psychological counseling and psychological intervention should be strengthened, especially psychological counseling for patients and their families, family members of the deceased. This is a fight against viruses and a psychological battle.

Often feel the limbs tremble for no reason, unable to sleep peacefully, in the ward of the medical team of Wuhan Tongji Hospital in the New District of Wuhan Tongji Hospital, the psychological state of a female patient causes the attention of the nursing team. "After she was infected with the new crown virus, she was worried about her illness, and worried about the phone call of the family. She was crying," said Pu Weidan, an associate professor at the psychological center of Xiangya No.2 Hospital, Central South University. Professor Liu's team has been paying close attention to her psychological and physical condition. The death toll in the ward has been such a way of encouraging confidence that it will enhance her trust in medical staff. After careful treatment by the two party team, the woman was cured and discharged in March 1st.

At present, with the sharp reduction of the number of cases of new crown virus infection, the focus of treatment now has the need to tilt the psychological support. Prior to this, the National Health Protection Committee dispatched 150 psychiatrists from Shanghai, Sichuan and Hunan to support Wuhan. The team headed by Wang Xiaoping, director of the National Center for psychosocial psychological research (Xiangya No.2 Hospital) and director of the Institute of mental health, Central South University, together with 50 counterparts from Hunan Province, have arrived in Wuhan for many days.

"The earlier the psychological intervention and psychological intervention is, the better the effect will be," said Li Weihui, an associate professor in the team specializing in post-traumatic stress disorder. "No matter the medical staff or patients, the more aware and timely identification of psychological stress response and the mastery of self psychological control methods, the less likely the future occurrence of PTSD will be."

The task of the national psychological medical team to rush to Wuhan is to help people with or without psychological stress and trauma to reduce or avoid psychosocial diseases such as post-traumatic stress disorder, anxiety disorder and depression.

"We have three roles: first, as a psychiatrist, to join in consultation with other medical staff, such as delirium of agitation or delusion or delusion, followed by one to one psychological support, including medical staff and patients; the third is training or science popularization of psychological support for a line of medical staff to let patients know which patients. You need us. " Wang Xiaoping explained the task of the team so much.

Psychological team helps Han

Before arriving in Wuhan, the Wang Xiaoping team has carried out psychological support related to the new crown pneumonia epidemic.

In January 31st, Xiangya No.2 Hospital, Central South University, Hunan mental health center and Central South University Mental Health Research Institute launched the "national response psychological assistance hotline platform" to the public and front-line medical staff on the joint psychological relief branch of the Chinese Medical Rescue Association (hereinafter referred to as the "chapter"). Then, the national anti epidemic psychological service platform was opened with Baidu encyclopedia. At the same time, Li Weihui also organized the psychological assistance manual such as "entering the shelter" and "getting out of the shelter" and carrying out the technical support for psychological assistance.

"So far, the national epidemic prevention Xiangya psychological service platform has served more than 2 million people." Zhang Yan, deputy director of the Institute of mental health at Central South University, said that most of the early consultants were either from Hubei or from new crown pneumonia patients. They either come to consult the disease knowledge, or are too anxious and suspicious because of the impact of the epidemic.

After the instructions arrived at the Han Dynasty, Wang Xiaoping's team refined the task to assist non psychiatric clinicians in solving clinical problems and doing psychological services for patients. The most important thing is to prevent major risk events.

Hospitalized patients in designated hospitals can not freely enter the ward. The psychological group will follow the medical group rounds according to the clinical needs. In view of the isolation ward environment, the psychotherapist Wang Jianjian and Liang Mining push the mindfulness training and relaxation techniques for the patients themselves. If there is an urgent need for intervention, the nurses and doctors on duty will use the mobile phone or remote network to link with the psychological group, understand the psychological state of the patients through video observation or conversation, and enter the ward at any time when necessary, and conduct on-site inquiry intervention.

There are a group of patients in the isolation ward who are especially afraid of sleeping. When they are in a nurse's ward round or a room door, they immediately sit up and show alertness. There are also a group of patients who will suffer from catastrophic cognition and feel anxious and anxious after the recovery. Some patients will regret and blame themselves, and feel that they are not good enough to have depression. But now, with the improvement of epidemic situation and physical condition, most people's emotional reactions will gradually improve.

In the shelter hospital, the common state of the patient includes anxiety, panic and insomnia. "They are concerned about the time of transfer and discharge, and how are they at home?" Gao Xueping, a specialist director, said that the mentality of this group of patients was relatively high in the shelter hospital. Patients with repeated positive nucleic acid tests showed more anxiety. "Medical means are hard to explain now. We can only reduce their anxiety by intervening in psychology."

Handle every case carefully.

Emergency treatment is also part of team work.

Third days in the Han Dynasty, the group headed by Professor Tan Liwen dealt with a special case. A 53 year old patient, who was excited after entering the cabin, did not wear a mask, did not sleep at night, and harassed other patients. The medical staff repeatedly persuaded him that his condition had not improved significantly. The Wang Xiaoping team listed them as the focus of observation and continued to follow up their psychological appeasement and support.

But one day, the patient suddenly became excited, lying on the floor crying, and refused to cooperate with the medical staff and police officers. When Gao Xueping got into the cabin, he found his patient knew that the patient had been very excited since he entered the shelter. He often talked big and threatened to let the whole nation know about him. He often exaggerated his health.

"In terms of performance, he is manic, but there are many situations that lead to this state and need to be identified. What should I do? " Gao Xueping said, at that time, it was impossible for her family to be contacted. She immediately went into the cabin for an on-site interview, and found that the patient was still in a state of flux. His behavior was obviously different from that of the ordinary people, and there was still a threat to others.

At this point, the team also started discussions in the rear area, considering that it is appropriate for "suspected mental disorders". Initially, the team tried to persuade him to cooperate with the treatment, but ultimately failed. In addition, the mental health law stipulates that patients with suspected mental disorders are injured or threatened by others. According to the regulations, they are advised to transfer the patient to the mental health institutions at the designated sites for observation and treatment. "But we believe that this kind of manic performance may also be related to stress, and can only be used as a temporary transition diagnosis, not a final diagnosis. Step observation, clear diagnosis, reasonable treatment. Gao Xueping said that after 10 days of hospitalization, the patients are basically recovered.

Pay attention to PTSD of medical staff

Generally speaking, the state of PTSD (PTSD) will gradually show up about a month after exposure to the epidemic. At present, the psychological support of medical staff is also the focus.

"They were very busy before, and now with the relief of stress, they will have leisure time to take care of their psychological feelings." Li Weihui said, but many people regard mental and psychological problems as negative marks. Moreover, the medical staff have high expectations for themselves. Their professional ethics and professional responsibilities make them feel ashamed to seek psychological help from their peers. This also brings the difficulty of psychological services for medical staff.

Among them, care teams deserve more attention. Associate professor Chen Qiongni also added that they are closer to patients, get along longer, and are younger and need psychological care. Now, two psychotherapists He Li and Tang Juanjuan have consulted patients and health care teams for 3000 minutes.

Tan Liwen said, "medical staff need to look at things from a positive and dialectical perspective, and use the cognitive mode that can play a positive role in comfort to improve their depression and anxiety, and further improve their behavior." Now, Dr. Huang Xiaojun and Dr. Chen Xudong are working with colleagues to help Bahrain medical workers through online technology.

In fact, psychological support intervention is not too late in the new crown pneumonia treatment. As early as January 26th, the state had urgently introduced the emergency principle of psychological crisis intervention. In addition, all kinds of psychologists have published various kinds of popular science books and propaganda tools for different groups of people. All provinces and municipalities are helping the medical team of Wuhan and the local psychological workers to serve the medical staff.

With the orderly development of all aspects of work, most mental problems will be resolved automatically, but some people will have the possibility of PTSD. "Compared with SARS, the occurrence of PTSD in survivors of the Wenchuan earthquake will be reduced by the state's emphasis on psychological crisis intervention." According to the literature and intuition, Dr. Luo Xingwei reported that the proportion of PTSD caused by new crown pneumonia would be less than SARS, but the base was large.

In response to this group of people, "after the outbreak, we can take the initiative to do some stress management training, so that the relevant personnel to participate voluntarily." Wang Xiaoping suggested that there are some early warning factors for PTSD. If we know the early warning factors or sources, it will be easier to intervene in the future. They will also formulate different methods, processes, plans and so on from different groups of people, different stages and different situations. In the future, it is also very hopeful to promote the development of some AI devices for psychological services under the epidemic situation.
